Visit Klaviyo homepage, create a Kalviyo account or log in to your account.
In your Klaviyo main menu, navigate to Signup Forms > Create Signup Form.

Here you can select one of pre-built forms to start customizing or design your own form by clicking on Build from Scratch.

Select desired form type. There are 3 types of Klaviyo signup forms:
Popups appear in the middle of the browser's window. A popup is attention-grabbing, and is best used for important offers or critical messaging. Popups have the highest conversion rate of any form type.
Flyouts slide into the browser's window from any direction you select. These, like popups, can be used to get someone to accomplish something or redirect them, but because it’s a less intrusive option, you can use it to gather information about your browsers in a passive way.
Embedded forms appear where you paste the provided code snippet on your site. Embedded forms are the least intrusive signup form and are typically used in the footer of a site to collect email addresses.

Enter Name and choose the Email subscriber list. All subscribers who submit emails via this form will appear in the selected list. Then, click Save and Continue to Design.

Start designing your form, then select form status and click on Publish to save.

In case your form is embed, copy the provided embed code.
